Rất tiếc là chúng tôi chỉ được biết số lượng và các loại máy cận ngày
cấy máy nên đă không thể loan tin sớm đến các BN đang có nhu cầu. Các
máy c̣n lại sẽ được dùng trong các chương tŕnh từ thiện sau này. Các
BN đă đến từ các tỉnh lân cận Kiên Giang (Cà Mau, Hậu Giang, An Giang)
và có thêm 1 BN từ B́nh Định.

Chúng tôi đă chăm lo các BN được cấy máy đầy đủ. Mỗi BN đă được
kiểm tra máy ngày hôm sau ca phẩu thuật, và nếu cần thêm lần
nữa,
để điều chỉnh máy hoạt động tối ưu.
BN B́nh Định là một trường hợp đặc biệt: BN nam <30 tuổi, đă
từng bị nhiều cơn loạn nhịp nhanh và bị ngất nhiều lần. BN từ một gia
đ́nh nông dân nghèo.Do sự can thiệp bác sỹ ở B́nh Định, chúng tôi đă
cho BN một máy phá rung, ICD, đă qua sử dụng chúng tôi đă xin được ở
Mỹ. Sau nhiều cơn loan nhịp với điều trị, máy đă hết pin. BS ở B́nh
Định đă nhờ chúng tôi giới thiệu vào chương trinh từ thiện này để được
thay máy. V́ các cơn loạn nhịp trước đây không được ghi lại rơ ràng,
chúng tôi đă không chẩn đoán được chính xác các cơn loạn nhịp (hầu đưa
đến điều trị bằng máy cắt đốt các ổ loạn nhịp). Do đó BN đă được nâng
cấp lên máy phá rung 2 buồng. Nhờ có các số liệu mới của máy 2 buồng,
sau một tuần phân tích các số liệu chúng tôi đă khám phá ra một nguyên
nhân có thể giải thích lư do tại sao BN mau mệt và đang bàn với Bs Huy
và Bs điều trị ở
B́nh Định cách để khắc chế sự kiện này. Về phương diện loạn nhịp, chúng
tôi đang chờ cơn loạn nhịp đầu tiến để có thể phân tích.

Defibrillators
at Kien Giang Provincial General Hospital

Vietnam
has cooperated with the inyerventional cardiology staff at Kien Giang
Provincial General Hospital, located in the city of Rach Gia, to
provide implant of free pacemakers and defibrillators. 23 devices,
including 14 pacemakers and 9 defibrillators were donated by Biotronik
USA. The estimated value of the equipment
provided by Biotronik USA (Lake Oswego, Oregon, USA) comes to 800 K USD.
Dr Huy M Phan (Arizona, USA) of Impact health and Dr Huynh Trung Cang
and his staff performed the implants.
Northwest
Signal Processing, Inc. provided technical support for the
implants, and
Systolic Medical
Products
provided equipment support.
The
single (11) and dual (3) chamber pacemakers and single (1) and dual (8)
chamber defibrillators are top of the line devices both in the US and
in Vietnam. All the pacemakers have CLS physiologic rate control, a
unique features of the top of the line Biotronik pacemakers, with
outstanding longevity, namely nominal 15 years for single chamber
devices and nominal 12 years for dual chamber devices. Much greater
longevities have been observed due to the efective automatic amplitude
control programs in the pacemakers. The defibrillators are equipped
with the largest capacity battery available in Vietnam, namely 1.72 Ah
and 1,73 Ah batteries providing outstanding longevity yielding nominal
service life of 11.5 years for the single chamber device, 9.6-10.5
years
for
the dual chamber devices. All the dual chamber defibrillators have the
Smart Detection program to discrimate between ventricular tachycardia
(therapy needed) and supraventricular tachycardia (no thererapy
desirable) with sensitivity (detection of ventricular tachycardia)

99% and specificity (detection of supraventricualr tachycardia)
>
94%.
A number of Ilesto 7 VR-T DX defibrillator, a best seller
for Biotronik in the USA, were provided, giving the cardiologists at
Kien Giang hospital the opportunity to be the first in Vietnam to
implant this type of dual chamber defibrillator that requires just a
single lead (see picture above). This is a unique technology only
available from Biotronik.
During
the week, 9 devices were implanted by Dr Huy Phan (Impact Health) and
Dr Huynh Trung Cang and members of his interventional cardiology staff.

The patients came from Kien Giang and the 3 provinces around Kien Giang
and from the province of Binh Dinh in central Vietnam.

Due
to the short time from when the Biotronik USA donation list was
communicated and the implant week, we were not able to get more
patients. The remaining devices have been turned over the the Biotronik
representative office in Vietnam to be used for future humanitarian
programs.
All the above patients had one or more follow-ups to adjust the device
parameters for optimal performance.
The
patient from Binh Dinh is a special case. This is a male, <30
years
of age. He had suffered a number of episodes of tachyarrhythmia, and
had numerous fainting episodes. Through his cardiologist in Binh Dinh,
we have donated a 2nd hand defibrillator (ICD) that was collected in
the US. After a number of episodes of tachyarrhythmia with
defibrillator therapies delivered, his ICD battery became exhausted.
His cardiologist in Binh Dinh has asked us to help the patient get a
replacement ICD through this program. Since his earlier episodes were
not correctly recorded, we were not able to accurately diagnose his
arrhythmia (with an eye towards giving an experienced
electrophysiologist enough information to ablate the arryhtmia focus).
For this reason, it was decided to upgrade him to a dual
chamber
ICD. With the data abtained from the dual chamber ICD, after a week of
analysis of the latest followup data collected in Kien Giang, we
believe we have been able to explain the cause of this frequent weak
spells. We are in discussion with Dr Huy and his cardiologist in Binh
Dinh on how
to overcome this latest problem. With regard to his tachyarrhythmia
episodes, we are still waiting for an episode so that we can diagnose
it accurately.
